CASE A
A 71-year-old woman with a mass discovered incidentally in the left parapharyngeal space. Ax, axial; CT, computed tomography; FS, fat saturated.

CASE B
Routine neck imaging for a 73-year-old man with a history of medullary thyroid cancer revealed a mass. Ax, axial; Cor, coronal; FS, fat saturated.

DESCRIPTION OF FINDINGS

  • Case A: Contrast-enhanced CT demonstrates a homogeneously enhancing ovoid mass in the left prestyloid parapharyngeal space. MRI demonstrates the ovoid mass to be sharply marginated with uniform intermediate signal intensity on T1-weighted images, intermediate to high signal intensity on T2-weighted images, and moderate enhancement on postcontrast T1-weighted images.

  • Case B: Axial and coronal contrast-enhanced CT demonstrates a mass in the left retropharyngeal space with a hypodense center and mild peripheral enhancement. The lesion is medial to the ICA.

Diagnosis

Case A

Pleomorphic adenoma

Case B

Cystic medullary thyroid carcinoma metastases in the retropharyngeal space
