Ear disorders are common in general, with earache being a frequent complaint. It may represent ear disease or referred pain from the oral cavity or pharynx. Causes of deafness are dealt with on p. 86, with many differentials overlapping.

External ear

  • Trauma

  • Boils

  • Dermoid/epidermoid cyst

  • Malignancy (e.g. basal cell carcinoma, squamous cell carcinoma)

External auditory meatus

  • Otitis externa

  • Boils

  • Malignancy

  • Osteomas/exostosis

  • Chondrodermatitis nodularis helicis

Middle ear

  • Acute otitis media (AOM) ( Fig. 16 )

  • Chronic otitis media

  • Mastoiditis

Referred pain

  • Dental problems (e.g. severe gum disease)

  • Pharynx

    • Tonsillitis

    • Pharyngitis

    • Foreign body

    • Quinsy

  • Carcinoma of the posterior third of the tongue

  • Laryngeal cancer

Neurological

  • Herpes zoster

  • Glossopharyngeal neuralgia
