Fenoldopam is a selective dopamine D 1 receptor agonist, which has been used in the management of acute severe hypertension and heart failure [ ], and to preserve renal function in various circumstances, such as after cardiac surgery [ , ]. Because it increases glomerular infiltration rate by renal vasodilatation, it has also been used in an attempt to reduce the risk of contrast-induced nephrotoxicity. The authors of a review of studies that used fenoldopam to prevent contrast nephrotoxicity concluded that it offers good protection [ ]. However, fenoldopam has not been consistent in offering good protection against contrast nephrotoxicity, it has to be given by intravenous infusion, and it requires careful monitoring of blood pressure and dosage adjustment if hypotension develops.
