54M with prior shoulder surgery now with L neck/shoulder pain and numbness. MRI with disc herniation. Please evaluate.

Initial Imaging

Fig. 36.1, Sagittal (A) and axial (B) T2-weighted MRI of the cervical spine without contrast demonstrates a left eccentric C6-C7 disc bulge (arrow) causing moderate foraminal stenosis without any evidence of canal stenosis.

Walking Thoughts

  • What is the patient’s neurological exam?

  • Are the symptoms limited to pain and numbness or are there any signs of weakness or myelopathy?

  • Is the patient’s prior shoulder surgery related to his symptoms?

  • Why did the patient choose to come to the hospital today if the symptoms have been ongoing for one month?

  • MRI shows a C6-C7 disc herniation—does the patient have a C7 radiculopathy which would fit with the imaging findings?

  • Has the patient tried any nonsurgical interventions yet?
