Hexetidine has been used as an oral cavity antiseptic and in preventing plaque and gingival inflammation [ ]. At the concentration normally used (1 mg/ml), it is effective in vitro against Gram-positive bacteria and Candida albicans , but insufficiently active against most Gram-negative bacteria. However, the duration of the reduction in germ count is not longer than 1 hour. When applied to mucosae it also appears to have a mild local anesthetic property.
In a double-blind randomized comparison of 0.1% chlorhexidine (Chlorhexamed), 0.1% hexetidine (Hexoral), and placebo mouth rinses in 90 subjects with gingivitis or mild periodontitis), the Approximal Plaque Index, the Bleeding Index, the Community Periodontal Index of Treatment Needs, the Gingival Index, and the Discoloration Index were measured [ ]. Chlorhexidine significantly improved the mean Approximal Plaque Index after 4 weeks and reduced the mean Bleeding Index and GI. Hexetidine significantly reduced the mean Approximal Plaque Index, Bleeding Index, and Gingival Index. Although there were also significant improvements in all measures with placebo, the active compounds were significantly more effective and equal to each other. In all three groups there was some increase in the mean Discoloration Index after 4 weeks.
