Fig. 49.1, Axial (A) and sagittal (B) T2-weighted MRI of the brain without contrast demonstrates a large hyperintense infiltrative brainstem lesion with associated hydrocephalus, consistent with diffuse intrinsic pontine glioma.

Walking Thoughts

  • How long has she been having symptoms?

  • What is her neurological exam?

  • Is she having any vision changes or cranial nerve deficits?

  • Does this patient need emergent surgery for management of herniation or hydrocephalus?

  • Will this require resection or biopsy only?
