  • Is there an intracranial bleed causing his symptoms?

  • Is the patient wearing a hard cervical collar?

History of Present Illness

A 20 year old male is brought in by emergency medical services (EMS) as a trauma after a motor vehicle accident. He was an unrestrained driver at a moderate speed and was T-boned on the driver’s side with airbag deployment. The patient had a brief loss of consciousness after which he was able to self-extricate. Per EMS, he was ambulatory at the scene but confused. On arrival to the emergency room, the patient is GCS14 and combative with several episodes of emesis. On secondary survey, the trauma team noted cervical spine pain to palpation.

Vital Signs

  • T 36.9°C, HR 90, BP 138/52, SpO 2 99% on room air
